The undersigned agrees and does hereby release all liability and claims in any and all matters including death and property loses or damage in connection with any activity or accommodation of the above-mentioned business. The undersigned agrees and does hereby release from all liability and hold harmless Urban Grind TV, it subsidiaries and any of its contractors or employees representing or related to the business above. UGTV reserves the right to make modifications to this agreement. Video, Photo, and Audio Release Form I, the undersigned, do hereby consent to the use by Urban Grind TV, Urban Grind Radio and its subsidiaries of my image, voice, or both, in the video, photograph, or audio recording described below regardless of whether these materials are used for advertising, publicity, or any other purpose on behalf of the Urban Grind TV + Urban Grind Radio its subsidiaries. I agree that all such photographs, video, and audio recordings and any reproductions thereof, and all digital files shall remain the property of the Urban Grind TV, Urban Grind Radio and its subsidiaries, unless otherwise noted. I understand that the Urban Grind TV, Urban Grind Radio and its subsidiaries are not responsible for unauthorized duplications or use of the images, videos, or audio recordings by third parties, on the internet or otherwise. In addition, I waive all claims to compensation or damages based on the use of my image or voice, or both, by the Urban Grind TV, Urban Grind Radio and its subsidiaries. I also waive any right to inspect or approve the finished photograph, video, or audio recording. I understand that this consent is perpetual, that I may not revoke it.
